This case demonstrates a 51 year-old male who presented in severe distress, and diagnosed with type A aortic dissection confirmed on trans esophageal echocardiogram. Arterial lines from both right and left radial arteries attached to different transducers showed an obvious discrepancy in volume and value. Discrepancy between pulses and blood pressures in both arms can be a very helpful hint in the diagnosis of this relatively rare but potentially fatal condition, especially if not recognized early. Students, residents and fellows are always taught to perform a thorough physical exam, including checking blood pressure and pulse in bilateral arms, especially when suspecting aortic dissection.
